In order to stay deployable, predeployment medical form needs to be completed by September 30th, 2015 and sent to our medical officer, Caren Prather.
1. Do your own fitness test and complete the following form.
2016 Fitness for Duty
2. Complete the first four pages of the following form and return it to Caren.
NDMS Physical Medical Fitness Affidavit 2016
In addition to the medical screening forms and the fitness for duty form, Caren also needs a copy of your immunization records.
These can scanned and e-mailed to or faxed to Caren at 1(520)243-7550.
